OPTN 5X: ≥ 5 cm with arterial enhancement and either washout or pseudocapsule (lesions that meet radiologic criteria but outside stage T2 disease such as lesion over 5 cm, involving major veins around liver...)


OPTN 5T: any residual tumor or perfusion defect, post local regional tx of a prior class 5 lesion (all TACE, RF ablation, cryo).


OPTN 5B: ≥ 2 cm and ≤ 5 cm with increased arterial enhancement AND either washout or pseudocapsule.

OPTN 5B-g: ≥ 2 cm and ≤ 5 cm with increased arterial enhancement and growth of at least 50% on serial examinations ≤ 6 months apart.


OPTN 5A: ≥ 1 cm and < 2 cm with increased arterial enhancement AND washout AND pseudocapsule.

OPTN 5A-g: ≥ 1 cm and < 2 cm with increased arterial enhancement and growth of at least 50% on serial examinations ≤ 6 months apart.

 

 

Increased priority for transplantation:

- One class 5B or 5B-g HCC; two to three HCCs of any class other than 5X

 

Ineligible for transplantation:

- Any class 5X HCC; more than three HCCs; any HCC over 3 cm if more than one HCC
